Job summary

Trinity Health Corp. seeks a Social Worker (MSW) to plan, organize, and implement social services for PACE participants and families in Wilmington, DE. The role includes case management, advocacy, and collaboration with a multidisciplinary team to support home and facility-based care.

The position requires a Master’s in Social Work, a current DE license, and at least two years of experience, with preference for elder care experience and familiarity with Medicare/Medicaid.

Qualifications

• Master’s degree in Social Work or equivalent.
• Active DE social work license required.
• Minimum 2 years of social work experience.
• Experience with multidisciplinary teams preferred.

Responsibilities

• Assess participants and coordinate care plans with IDT.
• Conduct intake, provide counseling and support.
• Serve as liaison with agencies and refer resources.
• Coordinate discharge planning and transitional care.
• Provide individual and family counseling.
