Qualifications

• 2 years Social Work with geriatric age groups from diverse ethnic cultures.
• 1 year work experience with geriatric population.

Responsibilities

• Provide social work services to participants with direct service activities and counseling.
• Assess and address social, emotional, cognitive and ethnic factors for an individualized care plan.
• Access community and financial resources and ensure continuity of care across the continuum.
• Conduct social work assessment and intervention in home and other care settings as needed.
