Job summary

Qualifications

• Current LMSW license in Texas; ACMA certification preferred.

• Provide crisis intervention and mental health assessment as part of discharge planning.
• Coordinate with physicians, case managers, and care team to ensure timely discharge.
• Advocate for patient and family empowerment to access post-acute resources.
• Educate patients and families about post-acute options and community resources.
• Maintain safety, quality, and compliance with Memorial Hermann policies.
