Job summary

Memorial Hermann Health System is seeking a Licensed Master Social Worker to deliver clinical social work and complex discharge planning for patients with high psychosocial needs. Under supervision, you will provide crisis intervention and coordinate multidisciplinary discharge plans with the care team.

Qualifications

• Graduation from an accredited Master of Social Work program (MSW).
• Current Texas LMSW license; ACM certification preferred.
• Experience in health care settings, especially inpatient social work.
• Strong communication and psychosocial assessment skills.

Responsibilities

• Assess psychosocial risk factors for patients and families.
• Develop and implement multidisciplinary treatment plans.
• Provide crisis intervention and mental health assessment as needed.
• Lead and participate in discharge planning and post-acute referrals.
• Collaborate with physicians, case managers, nurses, and social work team.
