Job summary

HCA HealthONE Mountain Ridge in Thornton, CO is seeking a Social Worker (MSW) to perform non-clinical, biopsychosocial assessments and interventions on inpatient floors and in the outpatient emergency department. The MSW handles crisis intervention, patient/family support, high-risk screening, and referrals for financial or resource needs.

You will assist in post-acute care placement on all discharges with a focus on those with behavioral health needs.

Qualifications

• Masters Degree
• 1 year of experience or MSW internship

Responsibilities

• Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain a thorough knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with discharge planning or psychosocial needs.
• Coordinate and facilitate meetings between care team and patients/families/caregivers; support patient preferences and values.
• Intervene for psychosocial needs and support; provide counseling, advocacy, and referrals.
• Provide emotional support during end-of-life, terminal illness notifications, and crisis interventions.
