Qualifications

• Masters’ Degree in Social Work is required.
• Two (2) years experience in hospital or community social work is required.
• Case Management experience is preferred.

Responsibilities

• Perform bio-psychological assessments of patients.
• Develop and manage patient care plans for post-discharge services.
• Coordinate referrals to home health, DME, dialysis, short-term rehab, and nursing home placements.
• Serve as a resource for staff, patients, and families regarding community agencies.
• Assist with adoptions and special programs and education.
