Work Shift: Day - 8 Hour (United States of America)
The Social Worker is responsible for completing bio psychological assessments of patients and development of patient care plans for post discharge services. Additional discharge planning activities include, but are not limited to referrals to home health, DME, dialysis, short-term rehabilitation, and nursing home placements. Also, services as a resource for staff, patients, and families with regard to community agencies. Manages adoptions and assists with special programs and education.
Masters’ Degree in Social Work.
Two (2) years experience in hospital or community social work. Case Management experience.
None.
Knowledge of medical and social work terminology. Utilizes a variety of standard office equipment to include computers, printer, fax machine, CRT system, etc.
