Qualifications

• Master's Degree in Social Work required.
• Experience in discharge planning and hospital social work preferred.

Responsibilities

• Coordinates the discharge planning process for all patients.
• Evaluates psychosocial aspects to optimize treatment benefits and patient participation in care.
• Maintains knowledge of local, state and federal resources for patients and families.
• Ensures accurate data entry across information systems for HIM and Patient Accounts.
• Collaborates with the Care Coordinator/Interdisciplinary team to review the care plan and discharge status.
• Monitors progress toward discharge plan goals and revises as patient needs change.
• Transfers post-discharge care plans to the next level of care and ensures policy compliance.
