The role involves psychosocial assessments, care planning, resource provision, and documentation in the EMR, with ongoing emphasis on regulatory knowledge and community resources.

Qualifications

• Required Master's Degree in Social Work.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• MSW license in Michigan required upon hire.

Responsibilities

• Collaborate with interdisciplinary team to assist patients and families with complex psychosocial needs.
• Conduct psychosocial assessments and develop care plans.
• Coordinate with alternate care sites and discharge planning.
• Document assessments and plans in the electronic medical record.
• Provide consultation and resources to healthcare teams.
• Maintain knowledge of community resources.
• Participate in committees and projects as assigned.
• Stay updated on federal and state regulations related to the role.
