Job summary

Qualifications

• MSW required and permanent LSW licensure.
• CPI or alternative de-escalation training required within 6 months of hire.
• Experience with crisis intervention and mental health/substance use disorders.

Responsibilities

• Meet with patients and families to coordinate safe, timely services and education.
• Assess psychosocial risk factors and coping capacity.
• Counsel patients and families on emotional, social, and financial aspects of illness.
• Provide mental health assessments and crisis intervention as needed.
• Lead discharge planning and coordinate resources with community agencies.
• Act as liaison with protective services and respond to high-acuity situations.
