Job summary

IU Health Fort Wayne Hospital is seeking a Social Worker with MSW to provide face-to-face psychosocial support to patients and families, coordinating care with the clinical team to ensure safe, timely discharge planning.

The role includes crisis intervention, guardian/adoption/guardianship tasks, and serving as lead contact for abuse, neglect, and protective services across partnerships with APS and county teams.

Qualifications

• MSW degree required; LSW licensure required.
• Crisis intervention training or equivalent de-escalation training within 6 months of hire.
• Knowledge of psychosocial assessment, short-term counseling, and crisis intervention.

Responsibilities

• Meets with patients and families to assess needs and coordinate care.
• Collaborates with the health care team to ensure safe, timely care and discharge planning.
• Provides mental health assessments and crisis intervention as needed.
• Leads guardianship, foster care, adoption, and end-of-life support in designated cases.
• Acts as liaison with state and county protective services for abuse/neglect cases.
• Utilizes crisis intervention and de-escalation techniques in acute settings.
