Qualifikationen

• MSW from CSWE-accredited program; 3 years clinical social work experience at hire.
• Three years allied health practice with hospital/medical setting.
• TX Licensure as Master Social Worker at hire.

• Conduct psychosocial assessments and document findings.
• Assist in identifying decision makers and educate about patient rights.
• Provide liaison services and referrals to hospital resources and community agencies.
• Identify mental health needs and provide counseling/referrals.
• Collaborate with interdisciplinary team to coordinate care across continuum.
• Serve as resource on emotional/behavioral issues and refer to community services.
