Job summary

St. Luke’s Health–Brazosport Hospital in Lake Jackson, TX is seeking a dedicated Social Worker to advocate for patients and families facing complex challenges.

Qualifications

• MSW from CSWE-accredited program; 3 years clinical social work experience at hire.
• Three years allied health practice with hospital/medical setting.
• TX Licensure as Master Social Worker at hire.

Responsibilities

• Conduct psychosocial assessments and document findings.
• Assist in identifying decision makers and educate about patient rights.
• Provide liaison services and referrals to hospital resources and community agencies.

Job Summary And Responsibilities

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

• Conduct and document psychosocial assessment of patients
• Share responsibility in identifying appropriate decision makers if patient is without capacity, educating all in patients’ rights and encouraging participation in decision‑making and care
• Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs
• Identifies mental health needs of patients and Provides counseling and referral as indicated
• Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community by coordinating placement with home health agencies, long term acute hospitals (LTACs), skilled nursing facilities (SNF) and assisted living facilities, nursing home placement, acute rehab, psych placement, and custodial care facilitation
• Serves as a resource to the patient, family, and healthcare tem regarding emotional and behavioral issues and refers to community services available to adolescent, adults and elders

Required

• Masters Other from a Council on Social Work Education accredited graduate school of social work and Three (3) years clinical social work , upon hire or
• Three (3) years allied health practice, with hospital or medical background or experience
• Master Social Worker: TX, upon hire

Pay Range
