Job summary

St. Luke’s Health–Brazosport Hospital seeks a Social Worker to advocate for patients and families facing complex challenges, conducting psychosocial assessments, developing care plans, providing counseling and crisis intervention, and connecting clients with community resources.

The role requires strong interpersonal skills, empathy, and collaboration with multidisciplinary teams to ensure holistic care and patient empowerment across the continuum of services.

Qualifications

• Master of Social Work (MSW) degree from an CSWE-accredited program.
• Three (3) years clinical social work experience, to be met upon hire.
• Three (3) years allied health practice, hospital or medical background or experience.

Responsibilities

• Conduct psychosocial assessments and document findings.
• Share responsibility in identifying decision makers when patient lacks capacity and educate on patients’ rights.
• Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs.
