Saint Joseph London, part of CommonSpirit Health, seeks a Social Worker to advocate for patients and families facing complex challenges. You will perform psychosocial assessments, develop care plans, provide counseling, and connect clients to resources within a collaborative care team.

You will use specialized screening tools and create individualized plans, documenting progress while ensuring confidentiality and ethical standards.

Qualifications

• MSW required with demonstrated case management experience.
• Experience in discharge planning and transfer coordination.
• Crisis intervention skills and strong empathy preferred.

Responsabilités

• Conduct psychosocial assessments for priority patients.
• Outline strategies and services to address psychosocial needs and make recommendations.
• Use screening tools (depression, anxiety, substance abuse, health literacy, mental status, social support).
• Develop individualized psychosocial care plans with Case Manager - Acute Care.
• Document actions in medical records and handle information exchange with confidentiality.
• Provide crisis interventions and psychosocial counseling to restore well-being.
