Job summary

RWJBarnabas Health is seeking a Social Worker MSW to join our Ambulatory Clinic team in New Jersey. The role partners with physicians, nurses and allied health professionals to advocate for patients, assess needs, coordinate discharge plans, provide counseling, and connect families with community resources.

A Master’s in Social Work and NJ LSW/LCSW licensure are required, along with BLS certification and weekend rotation flexibility.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited School of Social Work.
• Active New Jersey Social Work Licensure (LSW or LCSW).
• AHA BLS Provider Certification required.
• Meets accrediting/licensing standards (CSW, LSW, LCSW).
• 1–3 years experience in related field preferred
• Flexibility to participate in weekend and holiday work rotation

Responsibilities

• Identifying and completing comprehensive clinical assessments for patients across assigned populations.
• Formulating discharge plans tailored to each patient’s needs.
• Providing counseling to patients and families regarding illness, coping, and transitions.
• Connecting patients with community resources and post-acute care with follow-ups.
• Participating in interdisciplinary rounds to review caseloads and advocate for patient needs.
• Coordinating social work services within the care team and facilitating meetings for goals and planning.
• Entering accurate clinical documentation into the patient’s EMR.
