Job summary

RWJBarnabas Health is seeking a Social Worker MSW to join our Ambulatory Clinic team in New Jersey. The role partners with physicians, nurses and allied health professionals to advocate for patients, assess needs, coordinate discharge plans, provide counseling, and connect families with community resources.

A Master’s in Social Work and NJ LSW/LCSW licensure are required, along with BLS certification and weekend rotation flexibility.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited School of Social Work.
• Active New Jersey Social Work Licensure (LSW or LCSW).
• AHA BLS Provider Certification required.
• Meets accrediting/licensing standards (CSW, LSW, LCSW).
• 1–3 years experience in related field preferred
• Flexibility to participate in weekend and holiday work rotation

• Identifying and completing comprehensive clinical assessments for patients across assigned populations.
• Formulating discharge plans tailored to each patient’s needs.
• Providing counseling to patients and families regarding illness, coping, and transitions.

As a Social Worker MSW, a typical day might include the following:

• Identifying, screening, and completing comprehensive clinical assessments for patients across assigned hospital populations.


• Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient’s unique needs.


• Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions.


• Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups.


• Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs.


• Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning.


• Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities.


• Entering accurate, clear, and timely clinical documentation and intervention records into the patient’s electronic medical chart.
