Job summary

RWJBarnabas Health's Clara Maass Medical Center is seeking a Social Worker MSW to support patients and families across the healthcare continuum. You will collaborate with physicians, nurses, and allied health professionals to ensure emotional and social aspects of care are championed.

Qualifications

• Required: Masters Degree in Social Work from an accredited school
• Current Licensure as a Social Worker in New Jersey
• Preferred: Licensed Clinical Social Worker
• Work Hour Requirements: Weekday Days and or Evenings and Weekend and Holidays

Responsibilities

• Identifying, screening, and completing comprehensive clinical assessments for patients across assigned hospital populations.
• Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient’s unique needs.
• Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions.
• Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups.
• Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs.
• Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning.
• Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities.
• Entering accurate, clear, and timely clinical documentation and intervention records into the patient’s electronic medical chart.
