Job summary

InnovAge Holding Corp. seeks a Social Worker with an MSW to plan, organize, and implement social work services for participants and caregivers in accordance with InnovAge policies and regulations.

The role is on-site at the center with field visits to participants' homes, providing psychosocial assessments, care planning, advocacy, and coordination with the interdisciplinary team to support aging in place.

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Current state driver’s license and dependable transportation.
• One year of health care experience; MSW preferred; strong psychosocial assessment skills.

Responsibilities

• Conduct psychosocial assessments and participate in care planning.
• Serve as primary liaison between participant/family and IDT; coordinate care, housing, and transitions.
• Complete in-home visits and follow-up assessments; participate in discharge planning.
