Job summary

AdvisaCare Home Health & Hospice is hiring a Medical Social Worker (MSW) to support patients and families facing life-limiting illnesses in the Gaylord, Cheboygan, Charlevoix, Traverse City area and surrounding communities.

The MSW will provide psychosocial and bereavement support, participate in IDT conferences, and help develop personalized care plans with the hospice team. Flexible part-time/per diem schedules are available to fit your needs.

Qualifications

• MSW degree from a CSWE-accredited social work program.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid driver’s license, car insurance and reliable transportation.

Responsibilities

• Performs psychosocial, emotional, spiritual and bereavement assessments and assists in developing and implementing goal-directed care plans.
• Provides care under the direction of a physician and participates in IDT conferences, sharing psychosocial, emotional, spiritual, physical and financial information.
• Documents all patient/family visits, calls and referrals in the patient record.
• Conducts reassessments of patient/family needs and counseling as required.
• Provides crisis intervention and individual or family counseling when indicated.
• Acts as a care advocate for the patient/family unit and maintains community referrals.
• Provides ongoing counseling related to death and dying to the patient and family as needed.
• Participates as a member of the Bereavement Team as assigned.
• Adheres to Hospice policies.

Education

Requirements

Job Duties

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
• Provides care under the direction of a physician. Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family
• Documents all patient/family visits, telephone contacts and referral actions timely, accurate and concisely in the patient record
• Conducts reassessments of patient/family needs and counseling as required
• Provides crisis intervention and individual or family counseling when indicated
• Plays an active role as a care advocate for the patient/family unit
• Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
• Provides ongoing counseling related to issues of death and dying to the patient and family as needed
• Participates as a member of the Bereavement Team as assigned
• Adheres to all Hospice policies
