Job summary

AdvisaCare Home Health & Hospice is seeking a compassionate Medical Social Worker (MSW) to support patients and families in Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding areas. This part-time/per diem role offers meaningful hospice work with one-on-one patient contact, flexible scheduling, and a supportive team.

Key duties include psychosocial assessments, care planning with physicians, bereavement support, documenting visits, and coordinating with community resources to ease

Qualifications

• MSW degree from a CSWE-accredited program.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid Driver's License, car insurance and reliable transportation.

Responsibilities

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in goal-directed care plans.
• Documents all patient/family visits, telephone contacts and referral actions in the patient record.
• Conducts reassessments of patient/family needs and counseling as required.
• Provides crisis intervention and individual or family counseling when indicated.
• Develops and maintains contact with community agencies to facilitate referrals and interagency cooperation.
• Provides ongoing counseling related to death and dying to patient and family.

Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• One year of experience in Home Health Care preferred
• Hospice experience preferred
• Valid Driver's License, car insurance and reliable transportation

Job Duties

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
• Provides care under the direction of a physician. Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family
• Documents all patient/family visits, telephone contacts and referral actions timely, accurate and concisely in the patient record
• Conducts reassessments of patient/family needs and counseling as required
• Provides crisis intervention and individual or family counseling when indicated
• Plays an active role as a care advocate for the patient/family unit
• Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
• Provides ongoing counseling related to issues of death and dying to the patient and family as needed
• Participates as a member of the Bereavement Team as assigned
• Adheres to all Hospice policies
