Job summary

Prominence Health is seeking a Social Worker to support transitions of care. Under supervision, you will partner with an interdisciplinary team to move members from hospital to home, ensuring safe, effective discharge and resource linkage.

You will handle high-risk referrals, educate families, and participate in TOC program activities, prioritizing regulatory compliance and quality outcomes.

Qualifications

• Master’s Degree in Social Work is required.
• At least 1 year of experience in medical social work, preferably hospital or hospice.
• Active LCSW in NV, TX, and/or FL; ability to obtain licensure in other states.
• Knowledge of Medicare/Medicaid processes and compliance.
• Excellent communication and writing skills; ability to work independently.

Responsibilities

• Collaborate with interdisciplinary care team to facilitate transitions from acute/post-acute to home.
• Engage with high-risk members to coordinate social services and resources.
• Educate members and caregivers about discharge needs and post-discharge plans.
• Organize support groups or counseling for members and families as needed.
• Ensure care aligns with TOC program workflows and regulatory standards.
