Job summary

Compassus is seeking a Home Health Licensed Master Social Worker to deliver coordinated psychosocial care in patients' homes across ages. You will assess needs, provide counseling, and refer families to community resources while aligning with Compassus values.

The position requires a Master’s in Social Work and LMSW licensure, with strong communication and teamwork. You’ll collaborate with care teams to ensure quality, compliant care and support patient and family throughout the care plan.

Qualifications

• Master’s degree in Social Work required.
• Licensed Master Social Worker (LMSW) in state(s) of employment required.
• Strong communication and interpersonal skills.

Responsibilities

• Provide psychosocial assessments, counseling, and referrals in the home health setting.
• Coordinate care with patients, families, and health care teams to meet needs.
• Document care plans, progress notes, and ensure compliance with payor requirements.
• Support quality patient care and promote Compassus values in daily practice.
