Job summary

Enhabit Home Health & Hospice is a large home-based care provider seeking a licensed Master of Social Work to administer social services and educate patients and families as part of a collaborative care team. The role follows physician orders and the established plan of care to support patients in the home setting.

Applicants should have a MSW, current state licensure, and at least one year of health care social work experience, with strong technology and documentation skills to coordinate care

Qualifications

• Masters degree from an accredited school of social work.
• Must be certified or licensed in the state where practicing.
• At least one year of social work experience in a health care setting.
