Scarborough Health Network in Canada is seeking a Social Worker with MSW to join the Medicine CP4 team. The role reports to the Manager of the assigned unit and supports diverse patients and families across hospital programs.

Qualifications include MSW and registration with the Ontario College of Social Workers and Social Service Workers, hospital experience, crisis intervention, discharge planning, and strong interpersonal skills.

Formación

• MSW degree and registration with the Ontario College of Social Workers and Social Service Workers.
• Experience in a hospital setting.
• Excellent crisis intervention skills.
• Strong focus on psychosocial assessments and discharge planning.
• Excellent assessment skills and familiarity with community services.
• Strong ability and interest in linking patients to community resources.
• Must be flexible and a strong team player.
• Demonstrates excellent interpersonal and communication skills and good judgement.
• Adheres to SHN’s Code of Conduct and Health & Safety policy.
