Job summary

University Health Network in Toronto is seeking a Social Worker to join the SCOPE Program at Toronto Western Hospital. You will support primary care practitioners and collaborate with psychiatrists to improve mental health care pathways.

The role emphasizes systems navigation, interprofessional teamwork, and guiding self-help approaches within recovery models. A MSW and Ontario SW registration are required, with 2–3 years of acute care experience preferred.

Qualifications

• Master's Degree in Social Work (MSW) or equivalent required.
• Registration with the Ontario College of Social Workers and Social Service Workers required.
• Membership in the Ontario Association of Social Workers preferred.
• 2–3 years' experience with an interprofessional team in an acute care setting preferred.
• Strong knowledge of mental health resources and prior mental health experience.
• Ability to work independently but seek consultation when needed.
• Excellent verbal and written communication skills.
• Excellent interpersonal, time management and organizational skills.
• Excellent problem-solving skills.
• Research experience/education an asset.
• Student education/supervision an asset.
• Knowledge of policies, legislation and issues related to social welfare institutions and services.
• Computer literacy preferred.

Responsibilities

• Assess patients to evaluate need for services, problems, and formulate treatment plans.
• Implement intervention plans with patients, families and the interprofessional team to facilitate changes.
• Facilitate family and team meetings; reassess and modify plans as needed.
• Liaise with community agencies.
• Complete applications for Alternate Level of Care facilities.
• Coordinate with interprofessional team and agencies for psycho-social info exchange.
