VitalCaring is seeking a Master’s Social Worker (MSW) for field-based Home Health. You will assess psychosocial needs, develop individualized care plans, provide counseling, and connect patients and families with community resources.

Work alongside nurses, therapists, and physicians to deliver holistic, patient-centered care. This role emphasizes autonomy, ongoing training, and a commitment to meaningful impact in patients’ lives.

Qualifications

• MSW required and licensed to practice in state of employment.
• 1+ year healthcare experience preferred (home health strongly preferred).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Provide patient and family support by assessing psychosocial needs and developing care plans.
• Connect patients to community resources and coordinate referrals.
• Collaborate with the care team to ensure holistic, patient-centered planning.
