Qualifications

• MSW from an accredited program and active licensure.
• 1+ year of healthcare experience, home health preferred.
• Current CPR certification and valid driver's license.

Responsibilities

• Assess patients’ psychosocial needs related to illness, environment, and support systems
• Develop and implement individualized care plans focusing on emotional, social, and practical needs
• Provide counseling and support to patients and caregivers
• Coordinate referrals to community resources and support services
• Maintain EMR documentation and communicate updates to the care team
