VitalCaring Group is seeking a Master’s Social Worker (MSW) for home health, field-based and patient-centered care. You will assess psychosocial needs, develop individualized plans, provide counseling to patients and families, and coordinate community resources to support home-based care.

The role emphasizes collaboration with nurses, physicians, and a broader care team, with a focus on compassionate, holistic support and care planning in patients’ homes.

Qualifications

• MSW from an accredited program and current licensure in the state.
• Active licensure to practice social work in Oklahoma as required.
• 1+ year of healthcare experience, home health preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Assess patients’ psychosocial needs and develop individualized care plans focusing on emotional, social, and practical needs.
• Provide counseling, guidance, and support to patients and caregivers.
• Coordinate referrals to community resources and support services; liaise with providers and agencies.
• Document care activities in the EMR and communicate patient needs clearly with the care team.
