Job summary

VitalCaring is seeking a dedicated Master’s Social Worker (MSW) for a field-based Home Health role. You’ll assess psychosocial needs, develop care plans, and provide counseling to patients and families in their homes, collaborating with nurses, therapists, and physicians to coordinate holistic, patient-centered care.

Ideal candidates have MSW and active licensure, 1+ year healthcare experience (home health preferred), CPR, and a valid driver’s license with reliable transportation.

Qualifications

• MSW degree from an accredited program.
• Active state licensure as required.
• 1+ year healthcare/home health experience preferred.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide Patient & Family Support
• Assess patients’ psychosocial needs related to illness, environment, and support systems
• Develop and implement individualized care plans focused on emotional, social, and practical needs
• Provide counseling, guidance, and support to patients and caregivers
• Assist patients and families with navigating resources, financial concerns, and care planning
• Connect Patients to Resources
• Coordinate referrals to community resources and support services
• Serve as a liaison between patients, families, and community agencies
• Build and maintain relationships with local organizations to support patient needs
• Collaborate with the Care Team
• Partner with nurses, physicians, and interdisciplinary team members to ensure holistic care
• Participate in case conferences and contribute to comprehensive care planning
• Support discharge planning to ensure safe and successful transitions
• Document & Communicate
• Maintain accurate, timely documentation in the EMR
• Communicate patient needs and updates clearly with the care team and referring providers
• Uphold professional standards and regulatory requirements
