Zusammenfassung

VitalCaring Group is seeking a Master’s Social Worker (MSW) for a field-based Home Health role focused on patient-centered care. You will assess psychosocial needs, develop individualized care plans, provide counseling, and connect families to community resources while collaborating with the care team.

Ideal candidates hold an MSW, active licensure, CPR certification, and at least 1 year in healthcare (home health preferred). A valid driver’s license and reliable transportation are required.

• Minimum 1 year healthcare experience, home health preferred.
• Current CPR certification.
• Valid driver’s license with reliable transportation and auto insurance.

Aufgaben

• Assess patients' psychosocial needs related to illness, environment, and support systems.
• Develop and implement individualized care plans focused on emotional, social, and practical needs.
• Provide counseling, guidance, and support to patients and caregivers.
• Coordinate referrals to community resources and support services.
• Collaborate with nurses, physicians, and interdisciplinary team members to ensure holistic care.
• Maintain accurate, timely documentation in the EMR and communicate updates to the care team.
