Acute Home Healthcare is seeking a Medical Social Worker (MSW) for field-based work across Osceola County. The role involves coordinated care planning, documenting progress, and collaborating with physicians, patients, and families to address social and emotional factors affecting health.

The MSW will participate in discharge planning, maintain essential community connections, and ensure compliance with Medicare Conditions of Participation. A strong team approach and initiative are required.

Qualifications

• Master's degree from a CSWE-accredited school.
• Bachelor's degree in social work, psychology, sociology or related field.

Responsabilités

• Serve as a resource to employees, patients, physicians and other providers.
• Participate in development of the plan of care.
• Prepare clinical and progress notes.
• Assist in understanding social and emotional factors related to health problems.
• Work with the family and discharge planning.
• Document all patient/family services per agency policy.
• Plan/coordinating social services within the agency.
• Maintain contact with community support groups and know Medicare Conditions of Participation.
• Report any accidents or incidents to supervisor.
• Participate in patient care conferences and coordinate with community resources.
