Job summary

Responsibilities

• Provide psychosocial assessments, analysis, counseling, and referrals to meet patient/family needs.
• Assess home health patients to identify psychosocial, financial, environmental, and community needs from POC and records.
• Promote agency philosophy and ensure quality of care; participate in continuing education.
