• Masters in Social Work required.
• State licensure required.
• Experience in psychosocial care for chronic illness patients preferred.

Responsabilidades

• Assess patients' psychosocial status and plan care in collaboration with the interdisciplinary team.
• Provide counseling to patients and families to support adjustment to dialysis and treatment goals.
• Support transplant referral processes and discuss advance directives with patients and families.
• Educate patients on treatment modalities and available support services.
• Coordinate with Insurance Coordinators and care teams to address financial aspects of care.
• Assist with discharge planning and referral to community resources as needed.
