Job summary

Fresenius Medical Care in Clovis, NM seeks a Social Worker to provide psychosocial assessments, counseling and patient education for dialysis patients (in-center and home if applicable). You will work with a multidisciplinary team to plan care, support treatment adherence, and assist with transplant referrals.
