Job summary

Fresenius Medical Care is seeking a Licensed Social Worker to provide psychosocial services to dialysis patients, collaborating with the interdisciplinary team to support rehabilitation and quality of life. The role includes patient education, care planning, and assisting with referrals and community resources to promote adherence and overall well-being.

You will work within FMCNA guidelines, addressing advance directives, transplant discussions, and ongoing support for patients and families

Qualifications

• Master's in Social Work (MSW) required.
• Licensed to practice social work in the state.
• 2–5 years of related social work experience.

Responsibilities

• Assess patients’ psychosocial status and needs as part of the care plan.
• Participate in care planning with the patient and healthcare team to identify interventions.
• Provide supportive counseling to patients and families as permitted within scope.
• Educate patients on transplant options and assist with referral processes.
• Coordinate with community resources for home health, vocational rehab, and other supports.
• Document MSW interactions and interventions per FMCNA policy.
