Job summary

Fresenius Medical Care is hiring an entry level Medical Social Worker to support psychosocial care for patients undergoing dialysis, including in-center and home dialysis where applicable.

The role collaborates with the health care team to promote adjustment, rehabilitation and quality of life, utilizing social work theory and established practices. Requires MSW and state licensure eligibility, with 0–2 years of related experience preferred.

Qualifications

• Masters in Social Work required.
• Must have state temporary license or licensure as specified by state guidelines.
• 0-2 years' related experience preferred.

Responsibilities

• Assess patients' psychosocial status and assist with care planning.
• Provide counseling and support to patients and families.
• Coordinate with the interdisciplinary team to address barriers to treatment adherence.
• Educate patients on transplant options and related services.
• Facilitate access to community resources and financial assistance as needed.
• Document interventions and patient progress per policy guidelines.
