Job summary

Fresenius Medical Care is seeking an entry-level Social Worker to provide psychosocial support to dialysis patients in Milwaukee, collaborating with the interdisciplinary team to promote adjustment and quality of life.

You will assess psychosocial needs, assist in care planning, and offer counseling and education to patients and families, including transplant information and access to community resources.

Qualifications

• Masters in Social Work and active state license are required.
• 0–2 years related social work experience in healthcare preferred.
• Strong communication, empathy and counseling skills are essential.
• Knowledge of dialysis patient education and local resources is a plus.

Responsibilities

• Assess patients' psychosocial status and identify needs affecting rehabilitation and treatment outcomes.
• Participate in care planning with patients and healthcare team to meet goals.
• Provide counseling and support to patients and families within licensure scope.
