Community Health Systems, Inc. is seeking a Licensed Master Social Worker - Care Manager at Merit Health Wesley in Hattiesburg, MS. This PRN role requires day shifts and coverage for both weekdays and weekends to coordinate discharge planning and transitions of care.

The Care Manager will review medical records, collaborate with the RN Care Manager, ensure compliance with standards, and maintain detailed documentation of referrals and patient interactions as part of a multidisciplinary team.

Qualificações

• Bachelor's degree in social work is required.
• Master's degree in social work preferred.
• 2-4 years of experience in an acute care hospital setting required.

Responsabilidades

• Coordinate discharge plans and transitions of care with the IDT.
• Review records for post-acute care needs and referrals.
• Refer cases to physicians or RNs when criteria are not met.
• Serve as liaison with community agencies and facilitate transitions.
• Document case management activities accurately and timely.
