Endeavor Health is seeking a Care Coordinator - MSW in Highland Park, IL. This per diem role supports patients with chronic illness through a multidisciplinary team and evidence-based care management.

The position requires a Master’s in Social Work and Illinois licensure as LSW/LCSW, with 1–2 years of acute care social work preferred. Compensation includes comprehensive benefits and professional growth opportunities.

Formación

• Master’s degree in social work required.
• Illinois licensure as LSW or LCSW; LCSW preferred.
• Licensed Social Worker (LSW) certification required.

Responsabilidades

• Collaborates with Continuum Care Managers, Case Managers and a multidisciplinary team to support patients with chronic illness and multiple co-morbidities.
• Adheres to established care management standards and evidence-based protocols.
• Coordinates between departments, clinics, and community resources to ensure safe transitions of care and manage utilization and cost of care.
• Uses social work methods for assessment, education, collaboration, and resources to improve patient self-efficacy and quality of life.
• Provides care coordination and counseling to Endeavor patients and value-based members.
