Endeavor Health in Glenview, IL seeks a Care Coordinator-MSW to support patients with chronic illness and coordinate across departments. The role requires an MSW and IL licensure, with 1–2 years of acute care experience preferred.

You will join a multidisciplinary team and contribute to high-quality outcomes. This full-time position offers Monday–Friday, 8:30 am–5:00 pm, with rotating weekends and travel between Glenbrook and Highland Park Hospitals.

Qualifications

• MSW required with Illinois LSW/LCSW licensure.
• 1–2 years of social work experience in an acute care setting preferred.
• Ability to communicate effectively and collaborate with diverse stakeholders.

Responsabilités

• Collaborates with care managers, physicians, and the multidisciplinary team to support patients with chronic illness.
• Adheres to care management standards and evidence-based protocols.
• Coordinates between departments and community resources to ensure safe transitions of care and manage costs.
• Applies social work methods to assessment, education, collaboration, and coordination of resources.
• Provides care coordination and counseling to Endeavor patients and contract members.
• Initiates Advance Care Planning and goals of care discussions when appropriate.
