Qualifications

• Master’s Degree in Social Work is required.

• Excellent communication and collaboration with diverse stakeholders.
• Ability to serve as a subject matter expert in care coordination.

Responsibilities

• Collaborates with Continuum Care Managers, Case Managers, Care Navigators, Physicians, Nursing, PCP and multidisciplinary teams.
• Provides care coordination across departments to ensure safe transitions and optimize total cost of care.
• Applies social work methods to assess, educate, and coordinate healthcare resources.
• Assists patients and families to gain self-management skills and improve quality of life.
• Participates in advance care planning and goals of care discussions when indicated.
