Elderwood Independent Practice Association (IPA) is seeking a Social Worker Care Manager who has an MSW to join our growing team of professionals.

You will conduct telephonic and in-person assessments for individuals with chronic illness or behavioral health needs, coordinate resources, and support care management across care settings. This role offers a Monday–Friday schedule with hybrid options and a starting salary of $67,000 per year, plus benefits.

Qualifikationen

• Master’s degree in social work is required.
• Minimum of one year social work experience with at-risk populations in home care, case management, discharge planning or managed care.
• Must have a valid NYS driver's license and good driving record.

Aufgaben

• Direct telephonic and in-person engagement with individuals with chronic illness or mental health issues and their families.
• Use assessment tools to determine member needs and link to community resources.
• Document in the EMR and report changes to the RN Care Manager.
• Participate in interdisciplinary team meetings and quality improvement activities.
