Salem Health in Salem, Oregon is seeking a Care Manager to screen admissions, coordinate care, and manage discharges. You will collaborate with a multidisciplinary team, provide emotional support, crisis intervention, and brief counseling to patients and families facing illness or new diagnoses.

The role includes leading care coordination to reduce readmissions, managing utilization review, and developing robust discharge plans with community resources.

Formación

• Master’s in Social Work from an accredited school of social work is required.
• Registration with the State of Oregon within 1 year of hire as either: Licensed MSW or Clinical Social Work Associate.

Responsabilidades

• Screen patients on admission to identify care needs and coordinate with nursing and multidisciplinary teams for care coordination and discharge planning.
• Provide emotional support, crisis intervention, and brief counseling to patients and families adjusting to illness or new diagnoses.
• Lead care coordination efforts to reduce readmissions and length of stay by collaborating with physicians, payers, and community resources; participate in complex care rounds.
• Manage utilization review processes, including hospital notices, securing financial resources, and assisting patients/families with Medicaid and community services.
• Develop and implement discharge plans addressing barriers and align resources with post-acute care needs in collaboration with the healthcare team.
• Perform psycho-social assessments and provide counseling, referrals, and interventions for abuse, addiction, and vulnerable populations.
