Qualifications

• MSW degree from a CSWE-accredited program is required.
• At least 1 year of Post-MSW clinical or medical experience required.
• Licensure options include LSWAIC, Licensed Advanced Social Worker, Licensed Social Worker Associate Advanced, or Licensed Agency Affiliated Counselor.

Responsibilities

• Provide developmentally appropriate care and plan for safe discharge and continuity of care for diverse patient populations.
• Advocate and educate; support patient/family self-care and management.
• Make Bioethics referrals and manage related processes.
• Educate physicians, staff and community partners.
• Coordinate case/care management education and training.
• Identify risks and coordinate referrals to resources.
