Minimum qualifications include a CSWE-accredited MSW, WA Clinical Independent Social Worker Associate License upon hire, and at least one year of SW

Qualificações

• Master's degree in social work from an accredited CSWE program.
• Washington Clinical Independent Social Worker Associate License required upon hire.
• 1 year of social work experience in a healthcare setting.
• 1 year of experience in acute care or completion of TIPs/Case Management Orientation Program.

Responsabilidades

• Assessment and planning, coordination of care and discharge planning for at-risk patients.
• Documentation of interventions and regulatory compliance.
• Advocacy and resource identification to address social, financial, cognitive/behavioral or legal barriers to care access.
