Qualifications

• Graduate of an accredited Physical Therapy program and Internship or rotation in appropriate clinical setting.
• Physical Therapist: AZ
• Basic Life Support - CPR

Responsibilities

• Perform initial evaluations, develop and update individualized plans of care, and provide direct treatments.
• Meet patient care productivity standards and collaborate with the interdisciplinary team and families.
• Participate in continuing education and develop advanced skills aligned with department goals.
• Complete documentation of treatment minutes and enter electronic charges accurately.
