Job summary

PIH Health Hospital is seeking a care navigator to support a statewide MOUD access initiative in the Los Angeles area. The role involves brief patient assessments, linking to MOUD programs, and expediting MOUD clinic appointments while serving as the primary coach and maintaining ongoing contact with patients.

Ideal candidates will understand SUD as a medical condition, demonstrate strong communication, and be committed to culturally competent care, including outreach to diverse communities.

Qualifications

• High School Diploma or GED.
• Minimum 1 year in substance abuse treatment services.
• LA City Fire Card within six months of employment.
• Preferred: Certification from a recognized certifying organization in substance abuse treatment.

Responsibilities

• Conduct initial brief assessments and link patients to MOUD clinics.
• Introduce patients to MOUD programs and services.
• Expedite appointments at MOUD-capable clinics.
• Serve as primary coach for clients and maintain ongoing contact.
• Assist access to financial counseling, primary care, mental health, social services, and residential treatment facilities.
