Job summary

Providence Healdsburg Hospital is seeking a Patient Navigator - Case Management to join the care team. This part-time role involves 8-hour day shifts and focuses on coordinating access to Medication for Opioid Use Disorder, conducting initial assessments, and linking patients to MOUD programs.

You will serve as the primary coach for clients, maintain ongoing contact, and assist with referrals to primary care, mental health, social services, and housing or residential treatment when needed.

Qualifications

• Bachelor's Degree in Social Work required.
• Master's Degree in Social Work preferred.
• California Licensed Behavioral Health Counselor or CA AOD Counselor Certification upon hire.
• CA Clinical Social Worker License upon hire.
• CA Marriage and Family Therapist License upon hire.
• At least 1 year of experience with patients with Substance Use Disorders.

Responsibilities

• Perform initial brief assessments and coordinate care.
• Introduce patients to MOUD programs and services.
• Expedite appointments at MOUD-capable clinics.
• Serve as the primary coach and maintain ongoing contact with clients.
• Assist with access to financial counseling, primary care, mental health services, social services, and residential treatment facilities.
