Allina Health, serving Minnesota and western Wisconsin, seeks an RN for outpatient clinic care in Lakeville. You will assess patients, plan and implement care, and coordinate with a multidisciplinary team to achieve optimal outcomes.

Responsibilities include data collection, nursing diagnoses, care planning, and ongoing evaluation. Strong emphasis on patient advocacy, safety, and continuous improvement in a supportive, values-driven environment.

Qualifications

• RN license required (MN; WI if applicable) or as dictated by state
• 0–2 years RN experience
• Current BLS certification (AHA or Red Cross) or in-house training

Responsibilities

• Assess, collect and synthesize patient data to identify care needs.
• Analyze data to determine nursing diagnoses and communicate with patient and team.
• Identify and set realistic, measurable outcomes with patient and family.
• Develop and implement an individualized care plan respecting patient needs.
• Coordinate care plan implementation safely and timely; document interventions.
• Evaluate patient progress and adjust diagnoses, outcomes and plan as needed.
• Engage in quality improvement and professional development activities.
• Provide ethical and collaborative care, guiding patients and families.
